Welcome to the "Telephone Consultation for International Travel" service of the Camino Medical Group (CMG) in Sunnyvale, California. This service is designed for international travelers who need professional travel medicine consultation services, but because of time constraints, or distance from our travel clinic, prefer a telephone consultation. If you are traveling internationally, we are now able to provide you with individualized travel medicine information, as long as you are a resident of California or reside outside the United States. Every attempt is made to minimize personal/medical information you give us over the Internet.

To initiate a travel medicine consultation, you must do the following:

1. E-mail rthompson@wellontheroad.com and give at least two preferred times for a return telephone call
2. List your office, home or cell phone #
3. State whether you are a "new" travel clinic patient or an "established" travel clinic patient of CMG
4. Give your CMG 6-digit chart number (if known)
5. If you are departing with very short notice and need to have a telephone consultation on an "urgent" basis, call the CMG International Travel Clinic at 408 733-4380 and ask for a "Telephone Consultation" with the travel medicine consultant.

At the time of the consultation be prepared to give our travel medicine consultant physician the following information:

1. Address, phone #, Date of Birth
2. Your CMG 6-digit chart number (if known)
3. Credit card information (no American Express)
4. Social Security Number
5. Pharmacy name and phone number
6. Immunization records (if known)
7. Current medications you are taking

Our consultant physician may ask you for additional detailed and specific identifying information. If you do not live in the Sunnyvale area, our consultant physician will help you find a health care provider in your area that is capable of administering the immunizations suggested at the time of your consultation. Our consultant physician is able to telephone your prescriptions to any pharmacy in California.

The basic charge for a standard telephone consultation, including medical record set-up, is $84 for the first 15 minutes. The only form of payment for this service is a credit card (no American Express). Telephone time more than 15 minutes will be billed at $3/minute. Experience shows most calls take 15 - 20 minutes. At the end of the consultation our physician will inform you of the time involved and the charge amount. We do not accept insurance payment for telephone consultations.
